The Code of Conduct is a form that all athletes and parents/guardians of athletes must sign and have in to the athletic office before participation in their sport. The form may be included in the Pre-participation Athletic packet, or seperately, found in the Athletic Office or the Health Office. Both pages MUST be signed by both the parent/guardian and student. Submit one copy with the required paperwork for a blue card and retain the other copy for your records. Submission dates for the 2009-2010:
- Fall Sports: Due on August 3rd, 2009
- Winter Sports: Due on November 6th, 2009
- Spring Sports: Due on February 22nd, 2009

Athletic Participation Forms
All students who participate in a sport must have an annual physical (valid for 365 days) with thier medical home (primary physician). The school physician is available only for those students without a medical home.
Subsequent sports will require all forms (listed below), minus the annual physical. All required forms are to be handed in to the Health Office according to the deadlines below. Once processed, a 'blue" card will be issued to the student. The blue card must be signed by the parent/guardian - no student will be allowed to practice without their blue card.
REQUIRED FORMS: In order to obtain a “blue card” the following must be completed & submitted to the Health Office:
1. High School Athletics Pre-Participation Physical Evaluation Form Both forms are include in this download, Part A: Health History Questionnaire & Part B: Physical Evaluation Form
2. MHS Athletics Steriod Packet
3. Athletic Code of Conduct - English or Spanish
4. Yellow Card: Emergency Card
5. Green Card: Athletic Participation Approval Form

NCAA Initial-Eligibility Clearinghouse
For Prospective Student Athletes
As a prospective student-athlete at a Division I or II institution, you have certain responsibilities to attend to before you may participate. Information concerning who needs to register with the Clearinghouse and what documents should be submitted; along with the initial eligibility clearinghouse process can be found at www.ncaaclearinghouse.net.
