Directions: to MHS home sporting event locations. If you feel that any directions can be made more clear or need to be changed please email the webmaster via the link to the right. Directions to Iron Hills Sporting Events: These directions were taken from the Iron Hills Conference direction booklet. Please email us to report any changes.
Spring Sport Announcement: All athletes planning to participate in the Spring season must get their paperwork in to the Health Office by February 22, 2008 in order to have your blue card in time for practice begninning in March (see below for additional information)

2007-08 Athletic Code of Conduct:
The Code of Conduct is a form that all athletes and parents/guardians of athletes must sign and have in to the athletic office before participation in their sport. The form may be included in the Pre-participation Athletic packet, or seperately, found in the Athletic Office or the Health Office. Both pages MUST be signed by both the parent/guardian and student. Submit one copy with the required paperwork for a blue card and retain the other copy for your records.

Athletic Participation Forms:
All students who participate in a sport must have an annual physical (valid for 365 days) with thier medical home (primary physician). The school physician is available only for those students without a medical home.

Subsequent sports will require all forms (listed below), minus the annual physical. All required forms are to be handed in to the Health Office according to the deadlines below. Once processed, a 'blue" card will be issued to the student. The blue card must be signed by the parent/guardian - no student will be allowed to practice without their blue card.

REQUIRED FORMS: In order to obtain a “blue card” the following must be completed & submitted to the Health Office:

1. High School Athletics Pre-Participation Physical Evaluation Form Both forms are include in this download, Part A: Health History Questionnaire & Part B: Physical Evaluation Form
2. MHS Athletics Steriod Packet
3. Athletic Code of Conduct - English or Spanish
4. Yellow Card:
Emergency Card
5. Green Card: Athletic Participation Approval Form

Important!

Effective January 1, 2008 - The New Jersey Department of Education has implemented a new Annual Athletic Pre-Participation Physical Examination Form; Part A & Part B.

This change will not affect any student currently on file; the change will be effective with any physicals done after January 1, 2008. Please discard any old Athletic Pre-Participation Physical Forms.

Deadline for Spring 2008 sports paperwork:
February 22, 2008

Deadline for Fall 2008 sports paperwork:
August 4, 2008
